This can definitely be an issue with small dogs.  The trick is to bend at the knees rather then the back and try to slow down the flow of treats so not rewarding as much.  If knees not so great either, you can get creative and use a long pvc pipe with a modified elbow at the bottom to make a cup.  Then, the client can drop a treat through the top and it will land in a cup.

If all these are not feasible, you can do a phase 2 like heel and just use a lot of praise when the dog is doing well and “free” the dog after small spurts and give a reward and some love.
