    Any word on her urine analysis?

    You mentioned that she is going out every 1-2 hours. How long does she get crated for other than at night? Can she hold it longer periods when crated or will she have accidents.
    3 accidents since we last talked seems like an improvement from going 2-3 times per week. Did you fill out a chart? Was their any trend in the time of day or activity as to when the accidents occur?

    From what you are telling me I am not sure that I would use a “Dog God” correction yet. It seems as if you have already been correcting her in a way that can cause anxiety and lead to insecurity. I understand the frustration with housebreaking especially the peeing on the couch, but this type of correction can definitely be counterproductive to housebreaking.
    I have seen dogs who did things like the couch incident, when they get anxious. Is it possible that she was anticipating a negative response from you when she got up to greet the kids? For example if she got excited to see the kids and you were directing her back to place.

    Have you used the crate as a punishment or emotionally? I always try to teach a dog to enter the crate on their own vs putting them into the crate. Make sure you are using high value treats and doing a few sessions per day where she does not have to stay in there. (5-10 reps of entering the crate and closing door with treats, but let her exit on last rep).
